NDIS and Complex Clinical care
Complex Home Care is provided by our Experienced Registered Nurses
Ongoing support at home
- Palliative care - pain and symptom control, end-of-life care and advance care planning
- Medication management
- Continence and bowel care, assessment and management
- Diabetes education and management
- Carer support and stress management plan
Home assessment
- Tailored nursing care plan
- Thorough health assessment
In home hospital type care
- Chemotherapy and Central venous access device (CVAD) care
- Ostomy care e.g Tracheostomy care and colonostomy.
- Acute and chronic complex wound care management
- Intravenous and subcutaneous medication management – e.g. IV antibiotics and subcut anticoagulant
- Nasogastric and gastrostomy feeding
- Catheterisation and ongoing care
NDIS services
- Personal care assistance (basic and complex)
- Pain management
- support coordination
- overnight care and respite care
Allied health support
- Follow up with your GP and pharmacist
- Information about referrals to allied health services such as physiotherapy, occupational therapy, podiatry and speech pathology
Personal care and assistance
Basic Personal Care is provided by our our Trained Health Support Workers
- Assistance for personal hygiene
- Assistance for activities of daily living
- One to one overnight support
- One to one 24 hour support
- Domestic home maintenance
